    Arts for Lawrence is made up of great people, volunteers, funders, donors, and sponsors working to bring the best performing and visual arts experiences and opportunities to the Eastside of Indianapolis. Upon approval, a scholarship may cover up to one week o f camp . Please fill out the application form and provide financial documentation. You will also be required to submit a letter of recommendation before approval of your applicati on. I understand and I agree In consideration of myself, my spouse (if any), or my child's or ward's participation at Summer Art Camp: I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY OR ALL ACTIVITIES ASSOCIATED WITH ARTS FOR LAWRENCE, and/or THIS FACILITY, THEATER AT THE FORT, OR ANY OTHER PART OF THE ARTS FOR LAWRENCE CAMPUS, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. In consideration of our application and permitting us to participate in this camp being held at Theater at the Fort, or any of Arts for Lawrence's other, facilities, we hereby take action for ourselves, our executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from negligence or fault of the entities or persons released, for our death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to us including our travel to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: Theater at the Fort, Arts for Lawrence, and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers; (B) INDEMNIFY, HOLD HARMLESS AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. We acknowledge that Theater at the Fort, Arts for Lawrence and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf. The risks include but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, and actions of other people including, but not limited to, participants, volunteers, monitors, and/or producers of the activity. These risks are not only inherent to participants but are also present for volunteers. The Accident Waiver and Release of Liability Form shall be constructed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I HEREBY GIVE MY PERMISSION FOR THE SUMMER ART CAMP STAFF TO OBTAIN THE SERVICES OF THE INDICATED PHYSICIAN OR HOSPITAL IN THE CASE THE NAMED CAMPER SUFFERS ILLNESS OR ACCIDENT. I HEREBY GIVE MY PERMISSION TO HAVE PICTURES/VIDEOS TAKEN DURING CAMP TO BE USED BY ARTS FOR LAWRENCE AND/OR THEATER AT THE FORT FOR ANY BROADCAST, PRINT MEDIA, GRANT DOCUMENTATION, OR OTHER PUBLICITY. BY TYPING YOUR FIRST AND LAST NAME BELOW, YOU ARE AGREEING TO ALL OF THE TERMS AND CONDITIONS OUTLINED IN THE RELEASE STATEMENT ABOVE. Find out how you can get involved! 2025 JUNETEENTH JUBILEE VENDOR REGISTRATION VENDOR/FOOD TRUCK REGISTRATION DETAILS (Please Read Before Filling out the Registration Form): Vendors: We accept food vendors, artists, and handmade/artisan vendors. No direct sales vendors. Cost: Vendor fee is $75. Space: Your vendor fee includes a 10x10 space. Vendors need to provide their own tables, chairs, and tents. Tents are not required but are strongly suggested. If space larger than 10x10 is needed, a second registration will need to be purchased. Food Vendors: Food vendors must be licensed with the Marion County Health Department. If you don't have a license, you will need to contact the MCHD to obtain an event license for that day. If you are a homemade food vendor, you will need to follow all guidelines set by the Marion County Health Department and obtain whatever licensing they require. Time: This year's event will be from 4-8pm. Vendors can begin set-up as early as 2pm, and will need to stay through the end of the event.
